Camp Health Form - Mandatory Form
This form is required to be on file for all campers for camp attendance.
Bunk Request Form
This form is used if your child wants to be in the same cabin with a friend.
Pen Pal Request Form
This form is used to obtain a pen pal for your child prior to camp.
Hotel Listing
A listing of hotels and lodging within 20 to 40 minutes of camp.

Horseback Riding Extra Program Form
This form is used to schedule more than 1 period of horseback riding instruction per week.
Quad Riding Extra Program Activity Form
This form is used to schedule the quad riding activity.
Dirt Bike Riding Extra Program Activity Form
This form is used to schedule the dirt bike riding activity.
Jet Ski Extra Program Activity Form
This form is used to schedule the jet skiing activity.
Scuba Clinic Registration Form
This form is used to register for the scuba clinic.
Rafting Trip Registration Form
This form is used to register for a rafting trip.
Private Tennis Lessons Registration Form
This form is used to schedule private, one-on-one tennis lessons.
Paintball Activity Registration Form
This form is used to schedule the paintball activity.

Free Clinics include: Cheerleading Clinic, Dee Jay Clinic, Skate Clinic, 5-Star Basketball Clinic, Circus Clinic, Rock Band Clinic, Hip Hop Dance Clinic, Equestrian Clinic, Horse Care Clinic, Beginner Golf Clinic, Competitive Tennis Clinic, Spy Camp, Survival Camp and Paintball Camp.

Spy Camp Release Forms
Paintball, quad riding and jet ski classes are included in the Spy Camp Program. These 3 classes however are optional and not required to receive the "Secret Agent Certification." Campers who want to participate in the paintball, quad riding or jet ski classes must have the appropriate release forms on file before they can be scheduled.
Paintball Camp Release Form
Campers cannot participate in this activity unless this form is completed and signed by a parent.
Equestrian Clinic Release Form
Campers cannot participate in this activity unless this form is signed by a parent.This release form is required for participation in the Equestiran Clinic.

Lake Lorraine Golf Trip Registration Form
This form is used to register for playing golf at a local course.
Scranton Yankees Baseball Games Registration Form
The 2012 registration form will be available later this spring.
